The 5 Steps of the Nursing Process (ADPIE)
1. Assessment : Collect subjective and objective data from the patient through interview, physical examination, and medical records.
2. Diagnosis : Identify the patient’s nursing problems using NANDA-I. Written in PES format: Problem + related to (Etiology) + as evidenced by (Signs & Symptoms).
3. Planning : Set SMART goals and write a Nursing Care Plan (NCP). Prioritise using Maslow’s hierarchy – physiological needs first.
4. Implementation : Execute independent, dependent, and collaborative nursing interventions. Document everything immediately.
5. Evaluation : Compare patient outcomes to expected goals. If goal not met, loop back to assessment and revise the care plan.
